Well...

# The dissection, started at the perineum area is made meticulously along the plane between the urethra/ prostate gland and the wall of rectum through the recto-vesicle pouch, which is the end of dissection as it close to the peritoneum (the boundary of abdominal cavity).

# The depth of the vaginal tunnel depends on the recto-vescicle pouch anatomy. Patients with large frame tend to get more depth than the ones with small frame. Anyway, my average is 6 inches depth.

# If you have had previous inflamation which cause scar adhesion in the pelvic, around the prostate gland or the wall of lower rectum, the dissection through the scar to reach the end of recto-vesicle pouch may become more difficult and the tunnel would possibly be shorter than the average.

# The created tunnel always needs its lining, which normally comes from the genital skin (only penile skin inversion or penile inversion plus scrotal skin graft).
If the two genital skin source is not enough for the total lining, the extra source from either the lower belly skin (my favorite choice) or the colon segment graft will be taken in consideration.

# A good skin or mucosa lining of neovagina can help prevent a naturally occuring wound contraction, which causes the vagina narrow or even collapse.

# To acheive the final satisfactory depth, it also depends on the patients' part to take their responsiblity to do vaginal dilatation diligently.
